Care built around you—not the other way around.
AdvoKate provides holistic healthcare via two platforms:
-
Healthcare that comes to you.
Getting to a medical office is not always easy-or necessary. AdvoKate offers in-home visits for patients who want personalized care in the comfort and convenience of their own home.
Home visits allow for more time, fewer distractions, and a better understanding of the factors that may be affecting your health day to day.
In-home care may be a good fit if you:
Have difficulty getting to a traditional medical office
Prefer the privacy and convenience of care at home
Need follow-up for an ongoing health condition
Are managing multiple medications or health concerns
Need a physical assessment but would rather avoid an urgent care or clinic waiting room
Have a family member or caregiver you would like involved in your visit
What can be addressed during a home visit?
Depending on your needs, an in-home visit may include:
Comprehensive or focused physical assessment
Blood pressure, heart rate, oxygen level, and other vital signs
Chronic condition follow-up
Medication review and management
Evaluation of new or worsening symptoms
Preventive health planning
Review of laboratory or imaging results
Coordination of referrals or additional testing when needed
Health education and individualized care planning
More than a house call.
Seeing you in your own environment can sometimes provide information that is difficult to capture during a traditional office visit—from how medications are being managed to mobility, safety, nutrition, caregiver support, and barriers that may be affecting your health.
The goal isn't simply to bring the exam room to your home. It's to understand the bigger picture and create a plan that works in your real life.
-
Quality care without the waiting room.
Not every healthcare concern requires an office visit. Telehealth gives you convenient access to your healthcare provider from home, work, or wherever your day takes you.
AdvoKate telehealth visits combine clinical care with something that can be difficult to find in traditional healthcare: time to ask questions, understand your options, and leave with a clear plan.
Telehealth works well for:
Chronic condition follow-up
Medication management
Blood pressure or diabetes follow-up
Review of laboratory or imaging results
Preventive health counseling
Follow-up after an urgent care, emergency department, or specialist visit
Selected minor illnesses and new symptoms
Treatment plan adjustments
Health education
Questions about your medications, diagnosis, or next steps
What happens during a telehealth visit?
Your visit is more than a quick video call.
We'll discuss what you're experiencing, review your health history and medications, assess what can appropriately be evaluated virtually, and determine the next step together.
Depending on your needs, that may include:
A treatment plan
Medication management
Laboratory testing
Additional diagnostic testing
Referral to a specialist
An in-person examination
Follow-up care
And if telehealth isn't enough?
Some symptoms simply need to be evaluated in person.
If your concern can't be safely or appropriately managed through telehealth, you'll be told that clearly and guided toward the appropriate next step. That may mean an in-home visit, laboratory testing, urgent care, a specialist, or emergency evaluation.
